Students should contact the Gymnasium Office for use of the facilities.   ● Kawauchi Gymnasium (map) ● Tennis Courts (map) ● Baseball Field (map) Report of Temporary Leave Outside of Japan If you have a plan to leave Japan temporarily to return to your country or travel overseas during your exchange period, you should obtain approval from your academic advisor and submit the form below.   Report of Temporary Leave Outside of Japan   Mandatory Medical Checkups　(Mandatory for all new exchange students) Regular medical checkups will be conducted at the Student Health Care Center (map) on Kawauchi Campus from April 22-26, and May 7-20. It is mandatory for all exchange students who started their exchange this Spring. It is optional for students who have been staying since last semester. Students are asked to make an appointment themselves using the online reservation system during the period between April 12 - May 7. The University-wide safety confirmation drill is held once every semester. All students are asked to respond to the safety confirmation message. Free Hotline: 0120 119 075 E-mail: [email protected] General Information National Health Insurance （国民健康保険） All exchange students enroll in the National Health Insurance. You should take your National Health Insurance card with you when you go see your doctor. By presenting your card, your medical costs at the hospital/clinic will be reduced 70% under the National Health Insurance Program.   You need to pay the insurance premiums using the payment slips, which will be mailed to your dorm address a few months after arriving in Japan. A monthly premium is about 1500-2000 yen for exchange students. You should pay it at the Japan Post Bank or 77 Bank in cash by the deadline indicated on each payment slip. For those who are staying only for the Spring Semester, you should pay the payment slips numbered 1 (第１期; payment deadline June 30) and 2 (第２期; payment deadline July 31). You can pay 1 and 2 at the same time. Do not pay the 3rd one (第３期; payment deadline August 31), keep it until you visit the Aoba Ward Office to deregister when you leave Japan, and ask them if you should pay for the 3rd payment slip as well. Click here for a payment slip sample.   For more details, visit the TU Support website.   National Pension （国民年金） All registered residents of Japan aged 20 to 59 years must join the National Pension System , and pay a premium every month. Exchange students apply for exemption from paying the national pension at the Aoba Ward Office. You will then not have to pay the pension. A few months after the procedure at the ward office, you will receive a letter and payment slips in the mail to your dormitory address asking you to pay the pension. However, you do not have to pay it so you can discard the letter. Click here for a sample of the exemption notice letter.   Social Security Number "My Number" Once you officially become a resident in Japan, a Social Security & Tax Number, commonly called "My Number", will be issued by the Japanese government. This 12-digit "My Number" is required on various occasions such as when opening your bank account, applying to extend your visa, etc. A "Notification Card" with your "My Number" on it will be sent to your address by registered mail. If you are not at home when delivery is attempted, your card will be held at your local post office for a limited time only. Please follow the instructions on the undeliverable mail notice to arrange for redelivery before the deadline expires. Exchange students rarely get asked to show their my number, but please keep the notification card safe during your stay in Japan. For more details, visit TU Support website.   Receiving a Delivery Notice in the Mail from the Post Office You will receive important mails, such as the My Number Notification Card, via registered mail. If the addressee is not present to receive the delivery, the Japan Post carrier will leave a delivery notice. You may wish to pick up the item yourself at a post office or have Japan Post redeliver it to your dorm. You will be asked to show your residence card for an ID check when signing.
